Service Notes Stroke Association offers practical support and advice for stroke survivors, helping to rebuild lives after stroke. When you leave hospital, our Stroke Association Support Coordinators will work with you to identify what support you may need to help you live the best life possible. The service can offer: Practical support • A full review of all your support needs • Hospital and home visits • Information about stroke and local services • Advice on reducing your risk of a further stroke Emotional support • Someone to talk to who understands • Tips and advice to support your recovery • Help to return to work or driving • Meeting other stroke survivors Communication Support • Group support from trained volunteers • Communication practice in a supportive environment • Information about aphasia and other communication difficulties • The chance to share experiences and make new friends
